Your controls. Providing health or injury information on our initial public enquiry form is optional. We do not use health information for marketing. We do not sell personal information. If you enquire about online or combined coaching, we only share your enquiry details with Ultimate Strength and Nutrition Pty Ltd when you have consented or otherwise authorised us to do so.

• Performance information: physical screening results, strength and power testing, force-plate data, swing information, assessment results and other information produced during coaching or testing.

• Health information: pain, injury, physical limitations, treatment, rehabilitation, exercise restrictions and recommendations from qualified health professionals that you choose to provide.

• Communications and media: emails, messages, feedback, uploaded documents and photographs or videos provided for coaching, screening or performance analysis.

• Booking and transaction information: appointments, attendance, services purchased, payment status and transaction records. Payment providers process payment-card details. Ultimate Golf Performance Pty Ltd does not need to store your complete card number.

• Website and technical information: IP address, browser or device information, website activity, cookies and security information collected when you use our website.

2. How we collect information

Providing health or injury information on the public enquiry form is optional. You may instead ask to discuss it privately with a coach.

Relevant health information may be required later during screening or onboarding so that we can assess training suitability, modify exercises and deliver coaching safely.

Ultimate Golf Performance Pty Ltd does not provide medical diagnosis or treatment. Our screening and coaching services do not replace advice from a qualified medical or allied health professional.

We may recommend that you obtain medical or allied-health clearance before participating or continuing with particular activities.

We do not use health information to target advertising or promotional messages.

5. Children and dependants

A parent, legal guardian or appropriately authorised adult may provide information for a child or dependant.

The person supplying the information must be authorised to do so and must provide accurate information relevant to the participant’s coaching, screening or training.

Where appropriate, we may require additional parent or guardian consent before collecting health information, recording photographs or video, conducting assessments or providing services to a minor.

• Ultimate Golf Performance Pty Ltd coaches, staff and contractors who require it to provide or administer services.

• A parent, guardian or authorised representative involved in the participant’s coaching.

7. Online and combined coaching enquiries

Ultimate Golf Performance Pty Ltd provides face-to-face coaching, physical screening and performance-testing services.

Online coaching and app-based services are provided through Ultimate Strength and Nutrition Pty Ltd.

If you select online or combined coaching and provide consent, Ultimate Golf Performance Pty Ltd may share relevant enquiry and contact details with Ultimate Strength and Nutrition Pty Ltd so that Ultimate Strength and Nutrition Pty Ltd can contact you and assess the appropriate online coaching pathway.

Ultimate Golf Performance Pty Ltd will not share an in-person-only enquiry with Ultimate Strength and Nutrition Pty Ltd for promotional purposes unless you have consented or separately asked us to do so.

Once Ultimate Strength and Nutrition Pty Ltd collects or receives personal information for its own services, its handling of that information is also governed by the Ultimate Strength and Nutrition Pty Ltd Privacy Policy.
